WITHDRAW FROM A CLASS
In order to withdrawal your athlete from our class schedule, please note the following:
1.
This form MUST be filled out and turned in to CCE no later than the 15th day of the month prior to the month of cancellation to avoid being charged for the next month.
2.
If this notification is not received by CCE by the 15th, the athlete's gym account will be charged for the following month.
3.
You are responsible for payment for your student's classes WHETHER OR NOT YOUR STUDENT ATTENDS CLASS until the time that you notify the staff via this form. Please do not rely on the student to verbally let us know that he/she will no longer be attending classes.
